ICRME Membership Dues ($30 with nametag, $25 without nametag).  Include the name needed on the name badge.  The membership form can be obtained at ICRME MEMBERSHIP APPLICATION.  Then either mail to:  Treasurer, P.O. Box 2108, Denver, CO  80202 or Email to:  ICRME Treasurer
